In order to enroll your child, please print and complete the application, and submit along with the following list of documents. You may submit your application by visiting in person, our office is located at 1205 Broad Street, Summersville, WV. (Behind Dairy Queen)
- Proof of Income
- Child’s Health Insurance or Medical Card
- Child’s State Certified Birth Certificate
- Child’s Immunization Record
- Child’s Physical Exam Form
- Child’s Dental Exam Form
- Proof of Physical Address (copy of utility bill, copy of personal tax receipt, each showing physical address)
- Custody/Court Documents (if applicable)
