In order to enroll your child, please print and complete the application, and submit along with the following list of documents. You may submit your application by visiting in person, our office is located at 1205 Broad Street, Summersville, WV. (Behind Dairy Queen)
2024_2025 Preschool Application
- Proof of Family Income (1 month of pay stubs, 1040 tax return, w-2’s)
- Proof of Residency
- Child’s Birth Certificate
- Child’s Immunization Record
- Child’s Health Insurance / Medical Card
- Child’s Social Security Card
