Weatherization Application Process
Nicholas Community Action PartnershipTo apply for weatherization services, please print and complete THIS APPLICATION, and submit along with the following list of required documents. (Your application is not considered complete without the required documentation below.) You may submit your application by visiting in person, our offices located at 1205 Broad Street, Summersville, WV.
- Copy of your most recent electric bill
- Copy of your most recent primary and secondary heating bill
- Application (click to print)
- Additional Household Member Information form
- Rental Release and Agreement form (If applicable)
- Zero Income Affidavit (If applicable)
- Photo ID (example: drivers license)
- Proof of home ownership
- DHHR SNAP Benefit Letter (if applicable)
- Copy of most recent electric bill with usage grid
- Zero Income Affidavit (if applicable, anyone over the age of 18 living in the household)
