Weatherization Application Process

To apply for weatherization services, please print and complete THIS APPLICATION, and submit along with the following list of required documents. (Your application is not considered complete without the required documentation below.) You may submit your application by visiting in person, our offices located at 1205 Broad Street, Summersville, WV.

• Proof of ALL Household Income. (if employed, 1 month of paystubs, 1040 tax return, w-2’s. If receiving Social Security, your Benefit Letter from Social Security.)
• Copy of your most recent electric bill
• Copy of your most recent primary and secondary heating bill

• Additional Household Member Information form
• Landlord Owner Investment form (if applicable)
• Rental Release and Agreement form (If applicable)
• Zero Income Affidavit (If applicable)
